Frequently Asked Questions

Q: Do you offer virtual visit/telehealth appointments?
A: Yes! When you call to make your appointment, we will ask if you preference is in office or telehealth. We utilize FaceTime or a Zoom account and will provide all the instructions necessary via phone or through our Phreesia check in system.

Q: How far in advance must I make my appointment?
A: Our practice offers same-day appointments, urgent care visits and scheduled preventative appointments.

Q: What should I bring to my first appointment?
We require a pictured ID and your insurance card. Also, please bring a complete list of current medications, any X-Rays, MRI and reports that may aid in your treatment. Reports and test results are a vital part of your treatment.

Q: Can I fill out my new patient forms before I come for my first visit?
A: Yes. If you are planning your first visit with us, please print and complete prior to your arrival. Please be sure to bring a copy of all current medications with the dosage as well as any allergies you may have. If you are unable to pint and/or complete forms patient forms prior to your appointment, please arrive at least 30 minutes before your new patient appointment.

Q: Can a minor patient attend an appointment alone?
A: No. A parent, guardian or adult must remain with the minor under the age of 18 years during the entire visit. Please do not drop the child off or leave the child in the office unaccompanied.

Q: How do I get a prescription refill?
A: If you call for a prescription refill, requests require 48 hours to be processed. However, we strive to have your request completed the same day if at all possible. Be sure to anticipate your refill needs so that they can be taken care of during your office visit. No medication refill requests will be filled by the on-call physician on Saturday, Sunday, or Holidays.

Q: Will the practice file my insurance claims?
A: Yes, we file all claims for our patients. However we do not file Third Party Liability claims for auto-accident claims. Payment for these Third Party Liability claims will be expected at the time Services are rendered.
